If you admit the allegations in the Charge and enter into a Consent Agreement, only Part A needs to be completed.
If you file a Reply and have the matter considered by the Professional Standards Committee, all parts of this form must be completed.

All Incident Reports filed with ITSO are processed following the procedures set out in the ITSO Professional Standards Complaints (PSC) Policy. Any information collected by ITSO at any time throughout this process may be used as evidence at a discipline hearing, if the matter proceeds to a hearing. You are required under the PSC Policy and REALTOR® Code to cooperate during PSC proceedings and produce any documents that are requested. Information in the documents produced, as well as any additional information that ITSO collects during the PSC process may include personal information as defined in the Personal Information and Protection of Electronic Documents Act. You consent to the collection, use and disclosure of personal information for ITSO to investigate this Incident and take any other action that is deemed necessary for enforcement of the ITSO Professional Standards Complaints Policy.
